Service Description
Follow-up visits typically last up to 30 minutes. These appointments provide space to reflect on how you’re feeling, make any needed adjustments to medications, and ensure your care continues to align with your needs and goals. Your provider will work collaboratively with you to support meaningful, ongoing progress. *Discounted rate for self-pay. Cost with insurance varies on individual plans. Each additional 15 minutes beyond the first half hour are charged $97.50.
Cancellation Policy
If you are ready to become a new patient, please begin by completing the questionnaire below. Based on your responses, you may be taken to a screen to schedule your first appointment or be notified that we will contact you for further information. You will receive an estimate for cost of services based on your insurance plan prior to your first appointment. You may cancel with no charge as long as you provide at least 24 hours notice prior to your appointment start time.
