Service Description
Intake evaluations are scheduled for one hour. During this time, your provider will focus on understanding your experiences, your values, and what kind of support you’re looking for. The goal is not to place labels, but to gain a clear picture of what’s going on so that care can be tailored to you. You’ll be invited to share openly, and any questions or concerns—whether about treatment, medications, or the process itself—are always welcome. *Discounted rate for self-pay. Cost with insurance varies on individual plans.
Cancellation Policy
If you are ready to become a new patient, please begin by completing the questionnaire below. Based on your responses, you may be taken to a screen to schedule your first appointment or be notified that we will contact you for further information. You will receive an estimate for cost of services based on your insurance plan prior to your first appointment. You may cancel with no charge as long as you provide at least 24 hours notice prior to your appointment start time.
